How to fill the Form







  • This Application includes 5 sections.5th section is separated from 1st,2nd,3rd and 4th sections which contains document uploading methods.
  • Click on Submit button to submit the form right after 4th section (Other Information Section)
  • Use applicant's National identity card number
  • Use 5th section (Upload Documents section)to upload scan copy of the business registration,Payment slip and other certificates along with the NIC number
  • Make sure, fill all the required fields
